While watching a documentary on underwater births, Cobain envisioned a baby swimming underwater, chasing a dollar bill.He initially wanted a photo of an actual childbirth for the cover.

He later told The Guardian, “I asked them if I could work on the project. No one in the company could imagine that the group was going to give such a ball,but the album impressed me as soon as I heard a test copy.”

Fisher researched underwater birth photos in a library,but found them unsuitable for an album cover,deeming some too graphic. Thru discussions with the band, the idea of the baby chasing a dollar bill emerged.
